Predicting outcome after stroke: the role of aphasia.

چکیده
OBJECTIVE Very few studies have investigated the predictive value of functional outcome, social outcome and discharge destination in patients with cerebrovascular accident (CVA) with aphasia. The aim of this study was to verify whether aphasia is predictor for outcome in patients with stroke with aphasia. METHODS The study was carried out in 262 patients with primary diagnosis of CVA and aphasia, included over a 6-year period (2001-2007): 131 with and 131 without aphasia. Statistically significant variables at the univariate regression analysis were submitted to the multivariate analysis. Backward stepwise regression analysis was applied to predict final motor-Functional Independence Measure (FIM), effectiveness in motor-FIM, final cognitive-FIM score and effectiveness in cognitive-FIM and discharge destination. Independent variables were age, gender, aphasia, stroke type, stroke lesion size, comorbidity, bladder catheter, motor function, trunk control test, initial motor-FIM and committed caregiver identified on admission to rehabilitation. RESULTS Patients with aphasia had lower motor-FIM and cognitive-FIM scores both at admission and at discharge, if compared with those without aphasia. Effectiveness in motor-FIM and cognitive-FIM scores was also poorer in patients with aphasia. Seventy-seven per cent of patients with aphasia and 91.6% of patients without aphasia returned at home. In the multivariate regression analysis, aphasia was predictor of final motor-FIM (β = 0.15), final cognitive FIM (β = 0.72), effectiveness in motor-FIM (β = 0.17) and discharge destination (β = 0.20). CONCLUSIONS Aphasia is a predicting factor of outcome and it is the most important predictor of social outcome in patients with stroke with aphasia.

The Effect of Priacetam in Aphasia due to Acute Brain Ischemic Stroke: Clinical trial
Background & Aims: Stroke incidence is 15 million people world wide annually and one third of these patients suffer from aphasia. Aphasia is often associated with significant disability in patients. Costs due to cerebrovascular diseases play an important role in national health expenditures. We aimed to assess the effect of piracetam in treatment of aphasia. Methods: Patients with acute ischemi...
متن کاملWhy Is It Difficult to Predict Language Impairment and Outcome in Patients with Aphasia after Stroke?
One of the most devastating consequences of stroke is aphasia. Communication problems after stroke can severely impair the patient's quality of life and make even simple everyday tasks challenging. Despite intense research in the field of aphasiology, the type of language impairment has not yet been localized and correlated with brain damage, making it difficult to predict the language outcome ...
متن کاملUnusual Recovery of Aphasia in a Polyglot Iranian Patient after Ischemic Stroke
Introduction: Aphasia is a language disorder resulting from a lesion in the cerebral cortex. In this case report, we present a polyglot patient who recovered from aphasia by speaking his newly active learned language Case Report: A 69 years old male referred with acute onset right hemiparesis and global aphasia. After imaging, he treated with 75 mg r-TPA (0.9 mg/kg). After the fourth day of ...
متن کاملImpact of Bilingualism on Cognitive Outcome After Stroke.
BACKGROUND AND PURPOSE Bilingualism has been associated with slower cognitive aging and a later onset of dementia. In this study, we aimed to determine whether bilingualism also influences cognitive outcome after stroke. METHODS We examined 608 patients with ischemic stroke from a large stroke registry and studied the role of bilingualism in predicting poststroke cognitive impairment in the a...

Background: To determine whether it is possible to predict intravenous thrombolytic therapy (IVT) outcome after 3 months in acute ischemic stroke patients who are candidate to receive recombinant tissue plasminogen activator (rt-PA), before rt-PA administration based on their risk factors and some available laboratory results. Methods: We enrolled 118 ischemic stroke patients who were treated ...
